Ukraine: Clinical Examination of 90 Patients Who Died of Pneumonia

  • This is an interesting report that clearly suggest it is not swine flu A/H1N1. So, earlier feedback that it is hemorrhagic (viral?) pneumonia appears closer to the truth. The onset to death is around 4-7 days. The age groups of the dead are: 58% - 19 to 40 years old,  29% – 41 to 55 years old. The translation seems to suggest some removal of medical records by the police?? Not sure what is implied here. (If there is anyone out there who could look at the Ukrainian source article and advise, it would be much appreciated.)

    Health Minister of Ukraine Vasyl Knyazevich during the conference call asked the medical community with the request at this difficult time more vigilant in keeping medical records, to avoid charges of providing substandard care. “Already today, police removed medical records, so I ask you, so we did not have” cases doctors “carefully postavtes his duties, including on” paperwork. “I understand where difficult conditions are now physicians, patient value for life is every minute, but once again calls for the respect of all the formal stuff. V. Knyazevich noted that if removal of documents, the hospital should remain a copy of them to allow other medical specialists, ministries make their own conclusion.
     
    As analysis showed, with 90 dead people constituted the largest proportion of young people, working age: 52 people aged 19 to 40 years, 26 people died at the age of 41-55 years, other cases have been reported in the elderly and children.
     
    The symptoms are observed at different stages of disease – a fever with a temperature over 38 C, cough, respiratory disorders. When cough was characterized by negligible allocation phlegm or dry unproductive cough with blotches of blood. All the patients come to hospital on average by 3-7 days of onset, were in serious condition. Period of time from onset to death averaged from 4 to 7 days. In all patients during a hospital for signs of respiratory insufficiency of various degrees, which quickly rose and manifested accelerated respiration rate, shortness of breath and effectiveness of independent breathing. X-ray studies were performed on 1-2 day hospitalization. Most patients experienced a double-headed particles of lower lung lesion, followed by a trend towards total destruction.
     
    All the conclusions of Clinical expert commission MZ of Ukraine were discussed with experts from WHO. Ministry of Health prepared and implemented a unified form of information gathering, as well as algorithms for examination of patients and clinical protocols during the response to the epidemic of influenza and pandemic influenza A \ H1N1 in the territory of Ukraine.

    Is gold the new dollar? 
    News on Tuesday that the Reserve Bank of India, the country’s central bank, had     purchased 200 metric tons of gold between October 19 and 30 from the International Monetary Fund sent gold soaring to a new all-time high. 
     
    The yellow metal closed at $1,090 an ounce. And it set loose speculation that could easily push gold to $1,300 an ounce on the current trend. What’s set tongues wagging?
     
    The idea that the buy by the Reserve Bank of India could be a signal that the world’s central banks are buying serious amounts of gold to hedge against a further decline in the U.S. dollar. If India’s buying, the thinking goes, can China be far behind?
     
    China has been quietly buying gold for years, doubling its holdings over the last six years. But the country still holds only 2% of its reserves in gold. That’s short of India’s 6.2% of reserves in gold after this buy. But both figures are far short of the 60% average in Europe or the 77% of reserves that the U.S. holds in gold. 
     
    The global average for the percentage of reserves held in gold peaked at 32.7% in 1989. In 2008, after 20 year of selling by the world’s central banks, the global average was down to just 10.3%. There’s enough logic to this theory to make it a powerful force in pushing up the price of gold. 
     
    The U.S. dollar does, indeed, seem to be in a long-term decline. Many of the countries with large currency reserves such as China are heavily over-weighted toward the dollar and are known to be looking for alternatives. The currency alternatives to the dollar have their own problems ranging from low trading volumes to fiscal deficits at home.
     
    Gold, in contrast, looks like a stable store of value for the long term. Which may explain why it is rising not only against the U.S. dollar but against all of the world’s major trading currencies.  Since the beginning of September, gold has outperformed even such “strong” currencies as the euro and the Australian dollar.
     
    I don’t see the rally in gold ending soon. If you ran one of the world central banks, could you think of a good reason to put your faith in the U.S. dollar?

1918 RBD Polymorphism in Ukraine H1N1?

  • It is looking more and more like the 1918 Spanish flu strain. This is a killer with 2-3% mortality rate. The young and the healthy died within a day of hemorrhaging lungs: “Alive and chirpy in the morning! Dead in the evening!” It was so bad every family was affected. Coffins ran out! Let’s hope for the best but prepare for the worse. This report is still pretty much intelligent assessment/guesswork. We still need DNA sequences to confirm.

    The recent explosion of H1N1 cases in Ukraine (see map) has focused attention on sequences linked to the outbreak, especially those in the lungs of patients who developed a cytokine storm.  This hemorrhagic pneumonia has been described previously in other fatal swine flu infections, but that rapid increase in reported deaths in Ukraine has raised concerns that the virus is transmitting more efficiently, or is replicating at higher levels in lung tissue.
     
    These changes are frequently linked to changes in the receptor binding domain (RBD) in the HA protein.  Changes in this domain can affect affinity for receptors and also modify tissue tropism.  The recent     expansion of seasonal H3N2 with M2 S31N was linked to two changes in or near the receptor binding domain, S193F and D225N.
     
    Recent isolates from Sao Paulo, Brazil, collected from necropsy tissue from fatal cases had two changes at position 225.  Two of the isolates, A/    Sao Paulo/53845/2009 and A/Sao Paulo/53838/2009) had D225N (see list), the same change seen in seasonal H3N2. Interestingly, the swine H1N1 is a triple reassortant with flu genes from swine, humans, and birds.  The human gene is PB1 and it was acquired in swine infected with a human H3N2.  The initial isolates had three human genes, the H3 and N2 as well as the PB1.  Thus, the prior association of the human PB1 in isolates with human H3,may increase the advantage offered by D225N.
     
    However, two other isolates from Sau Paulo, A/    Sau Paulo/53225/2009 and A/Sau Paulo/53206/2009, collected from the lungs of fatal cases, had another change at position 225, D225G.  This polymorphism is more widespread and recent isolates have been found in Japan, Italy, and China (see list ).  Moreover this polymorphism has been found in two isolates from the 1918/1919 pandemic, A/New York/1/1918 and A/London/1/1919.  Thus, in 1918 the H1N1 virus usually had a D at position 225, but some of the later isolates had D225G, which parallels the data from the 2009 swine H1N1 isolates.
     
    These RBD changes in recent isolates from Sao Paulo, as well as the presence of D225G in sequences from 1918/1919 raise concerns that the swine H1N1 is adapting to its human host by acquisition of RBD polymorphisms.
     
    The explosion of cases in Ukraine, and     delays in the release of sequences from fatal cases in Ukraine is a cause for increasing concern.  Recent accelerations of deaths have been widespread across the northern hemisphere, raising concerns that receptor binding domain changes described above, as well as a third polymorphism at position 225, D225E, (see list) are gaining traction as the swine H1N1 adapts to human hosts.
     
    An update on the Mill Hill sequences and deposit of such sequences at a public database such as GISAID, where Mill Hill recently deposited sequences from Europe, would be useful.

    Polish Prime Minister Donald Tusk said Friday that his government won’t buy vaccines for swine flu that have not been properly tested or from producers who won’t take responsibility for possible side effects. Tusk told reporters that vaccine producers were pressuring governments to buy, but were also demanding that all responsibility and compensation for possible negative side effects fall upon government shoulders.
     
    “Today we are dealing with great pressure from pharmaceutical firms … we are dealing with expectations that hundreds of millions of zlotys (dollars) will be spent on vaccine while no one wants to guarantee that it has no side effects,” he said. He stressed that the few dozen swine cases in Poland have been mild and no deaths have been reported.

    Polish health officials said a military medical laboratory in Pulawy was testing samples taken in Ukraine from sick people. Poland is also sending face masks to Ukraine.

Many Italian Physicians Reject Swine Flu Vaccine!

    Italian family physicians refuse to prescribe the A/H1N1 vaccine for their patients, claiming that the risks of the vaccine outweigh its benefits. Latest figures revealed that swine flu has infected more than 540,000 individuals, claiming the lives of 30 in Italy.
     
    The country has ordered some 48 million doses of the A/H1N1 vaccine, enough to immunize 24 million of its population, and plans to start a mass vaccination program in the coming weeks. Italian media outlets have, however, claimed that many of these vaccines would be useless as the majority of Italians including the Deputy Health Minister Ferruccio Fazio and the Mayor of Rome Gianni Alemanno have decided against vaccination.
     
    According to a recent survey conducted, six out of every ten Italian family physicians do not prescribe the A/H1N1 vaccine for their patients particularly those with heart diseases. A member of the European Association for Quality in General Medicine(EQUIP), Gianluigi Passerini told L’espresso that he would not advise vaccination for his patients, adding, “This virus is not aggressive and there is no reason to force individuals to get immunized against the virus.”
     
    Mario Nejrotti, the director of torinomedica.com, similarly, stressed that healthcare workers should only be urged to get vaccinated based on the severity and spread of the virus. He added that a similar vaccine manufactured in the US in 1976 had contributed to a considerable number of cases with a severe neurological disease known as Guillain-Barré syndrome.

Possible Ukraine H1N1 Genetic Changes in RBD Raise Pandemic Concerns!

    Right now we know that many clinical specimens and viruses have been sent to one of the WHO collaborating centres for further study. We don’t know the results of those studies, and it will probably take a couple of days for the full analysis of those viruses to be available.
     
    The above comments are from Thursday’s WHO     press conference and raise concerns that the sequences from Ukraine (see map) have small changes which may include receptor binding domain changes.  Several such changes have been seen in previously published sequences and position 225 has been of concern.  That position was linked to the establishment of amantadine resistance in seasonal H3N2 and raises concerns that such a change in swine H1N1 could make the virus more virulent.  In seasonal flu, the change was D225N, and that change is in at least three swine flu sequences, including two from fatal cases in San Paulo, Brazil collected in August (see list here). 
     
    The same position has also changed to D225G, which was also found in other fatal cases in San Paulo, Brazil.  It is also in more recent (September) isolates from China (see list     here).
     
    The new polymorphism most widely reported in sequences in Genbank is D225E, which is common in recent sequences from Italy (see list     here). Thus, similar changes in sequences from Ukraine would be of most interest, and such changes have not been ruled out by WHO statements thus far.
     
    The release of these sequences from Ukraine is now     overdue, and these delays increase pandemic concerns.

Slovakia Tightens Border With Flu-Hit Ukraine!

Slovakia said on Saturday it would close all but one road border crossing with Ukraine on Sunday because of Ukraine's flu epidemic.

  • I see this as a good move by the Slovakian authorities. This is not to sow panic but rather precautionary. We still do not know a lot of facts on the ground. Until we do, it is wiser to be safe than sorry. Having said this, we must be highly critical of all reports but keep an open mind. Let us all hope that this turns out to be another hoax and fear mongering by the PTB. Although, I really doubt it. Reuters reports:
     
    BRATISLAVA, Nov 7 (Reuters) – Slovakia said on Saturday it would close all but one road border crossing with Ukraine on Sunday because of Ukraine’s flu epidemic. Vysne Nemecke, the biggest checkpoint, will be the only one to stay open.
     
    “The reason is the continuing worsening of the epidemic situation in Ukraine,” Interior Minister Robert Kalinak told journalists. “This is intended to minimize any eventual outbreak of the disease.” Kalinak said the EU member state’s decision was temporary and aimed to protect the bloc.
     
    Doctors and hygienists will be stationed at the border to monitor passengers and have the powers to bar anyone suspected of being infected from entering the country.
